Seeking Input from Virginia Public Health Community

The Institute for Public Health Innovation (IPHI), with funding from the National Network of Public Health Institutes, is organizing a Virginia state forum to examine the understand of the Community Health Worker role in bridging health care and population health in Virginia.

The one-day forum, tentatively planned for March 2015, will bring public health and health service providers in Virginia together to define population health and explore the role of Community Health Workers in promoting population health and health equity within our health care systems and communities. It will also explore opportunities to expand the Community Health Worker workforce, as we strive towards improving the overall health of our state. IPHI is seeking input from the Virginia public health community to inform planning of the state forum. If you are a member of the VA public health community, please fill out this survey this survey. The survey will be open until November 14. An analysis of the results will be made available at the forum and electronically.  For more information, email fnageer@institutephi.org.
